Advanced Nursing

Advanced practice is a level of practice, rather than a type or specialty of practice. Advanced practitioners are educated at master’s level in #advanced_practice and are assessed as competent in practice, using expert knowledge and skills. They have the freedom and authority to act, making autonomous decisions in the assessment, diagnosis and treatment of patients. The concept of #advanced_nursing_practice is still evolving even as the #nursing #profession evolves. Although the term advanced nursing practice has been used extensively to describe the practice of the CNS, it is difficult to find a clear definition of the term.

They are offering new ways of delivering cost-effective care and increasing access to qualified practitioners for many patients and their families. They are often providing health care to under-served populations. Advanced practice nurses must be grounded in theory and research as guides to their #clinical practice. While they work in collegial capacities with physicians, they must be prepared to diagnose and treat patients with acute and chronic illnesses and to prescribe medications. These responsibilities require nursing professionals who are as smart and savvy as they are caring and compassionate.

Advanced Practice #Nurses perform these general functions:

• Obtain health histories and perform comprehensive physical examinations, including psychosocial, functional, and developmental assessment
• Order and interpret lab results and other diagnostic studies
• Develop differential diagnoses
• Develop/order therapeutic plan of #care
• Maintain patient records
• Evaluate patient’s response to plan of care and modify as needed
• Provide patient/family counseling and education
• Arrange for patient referrals/consultations
• Participate in research studies

The role has been articulated as a constellation of sub roles: clinician, educator, researcher, and consultant. However, describing various sub roles does not capture the broader picture of the CNS’s contribution to health care delivery. In a rapidly changing people-centered healthcare environment the advanced nurse practitioner can make an important contribution to healthcare delivery. The challenges ahead are many, as the advanced #nurse_practitioner requires policy and appropriate educational preparation to practice at advanced level. This will enable the advanced practitioner articulate the role, to provide expert client care and to quantify their contribution to health care in outcomes research. Advanced Practice Nurses are critical for the future, provide cost-effective care and are highly regarded by #patients/clients. They will be a constant and permanent feature of future health care provision. However, clarification regarding their classification and regulation is necessary in some countries.

Nurses can make a difference when it comes to malnutrition

For many people who already have had a decline in muscle mass associated with #aging, the addition of disease or injury leads to further loss of lean body mass and decline in functional abilities.

Up to 50% of hospitalized patients are at risk for or already have malnutrition, but only 7% actually receive a #diagnosis during their hospital stay, according to an article in #Clinical Interventions in #Aging and the Agency for #Healthcare #Research and Quality. #Malnutrition diagnoses can significantly impact #Medicare reimbursement and the case mix index for a facility. Patients who are malnourished may not only have longer lengths of stay of up to four to six days, but also hospital costs twice as high for patients with malnutrition, according to the Agency for Healthcare Research and Quality and International Journal of Environmental Research and Public Health.

According to the Agency for Healthcare Research and Quality, malnutrition also can lead to:

• An increased risk of adverse events and complications
• A 54% higher likelihood of #hospital 30-day readmission
• Up to 5 times more likely to die in the hospital than a non-malnourished patient

It was difficult to diagnose malnutrition in the past as there were no widely accepted characteristics that clinicians could use to classify this condition consistently throughout all care settings.

Nurses play a critical role in infection control in home healthcare settings

#Community #health #nursing – service rendered by a professional #nurse with communities, groups, families, individuals at home, in health centers, in clinics, in schools, in places of work for health promotion, #disease prevention, care of the sick at home and #rehabilitation. Community nurse beliefs about infection prevention influence whether they comply with prevention protocols more than their actual knowledge of how to comply, according to new US research. Based on these findings, the researchers suggested that altering perceptions about infection risk among staff was a promising strategy for improving compliance with infection control procedures. The team of researchers from #Columbia #University and the Visiting Nurse Service of New York surveyed 359 home healthcare nurses at two large agencies in the north eastern United States. It suggested that infection control strategies should include targeting the perception of workers by challenging existing beliefs – a tactic that has proven effective in promoting hand hygiene, they said. Among the findings were that only 68.5% of nurses in the sample agreed that the #influenza #vaccine was safe. Similarly, only 60.4% of nurses said their agency employer made it easy for them to stay home when they were sick.

These results suggested that home healthcare agencies may benefit from better educating staff on the influenza vaccine and employee sick leave policies, noted the researchers. While the survey results suggested that respondents had adequate knowledge about standard precautions to prevent the transmission of infection and protocols for handling exposure to bodily fluids, respondents were less knowledgeable about nursing bags and hand hygiene practices. For instance, more than a quarter of the #nurses surveyed failed to identify that hand hygiene should be performed after touching a #nursing bag. In addition, nearly 90% did not recognize that a nursing bag should have at least three separate compartments to segregate multi-use items from single-use items, and that hand hygiene supplies must be stored in a separate part, so hands could be cleaned before entering other compartments. These gaps in knowledge are notable considering that that the nursing bag could potentially serve as a vector for transporting infectious pathogens between home #healthcare #patients, said the researchers. They added that all survey respondents reported compliance with wearing gloves when anticipating contact with body fluids or blood products.

Nurses – Continuing Professional Development & Stressful Holidays for Nurses

Learning, #growth and #development do not end upon graduation from #nursing school. Continuing education plays a crucial role for nurses. #Nurses should plan to take at least one CE activity a month. Regularly taking #CE courses not only keeps your career knowledge up to date, it puts you on the path to lifelong learning, a critical component of being a #nurse.

Remember that ‘#Practice means any role, whether remunerated or not, in which the individual uses their skills and knowledge as a health practitioner in their profession. Practice in this context is not restricted to the provision of direct clinical care. It also includes using professional knowledge (working) in a direct nonclinical relationship with clients, working in management, administration, education, research, advisory, regulatory or policy development roles, and any other roles that impact on the safe, effective delivery of services in the profession.’ So that means that you can attain your #CPD hours via formal and informal activities. The type of learning activities selected can be broad and varied.

You need to have documentation of the identified learning need, a learning plan, your participation in the learning activity, and the outcome achieved will form the evidence of CPD you may need to provide. References to the articles that you have read are needed for self-directed activities.

#Healthcare providers aren’t the only ones separated from family during #holidays. Inpatient stays don’t magically end on the day before a special celebration, and thousands of #patients end up stuck in the hospital. Moreover, some patients may not have any family to speak of, thus they may see you as their substitute family. Giving a little extra and spreading holiday cheer to your patients is your sacred duty and privilege. In the end, cultivating compassion regarding our own feelings and the feelings of others is the key during the holidays. We need to remember that one person’s holiday may be another’s nightmare, and we cannot easily know how this time of year may manifest emotionally for any individual.

#Kindness, #self-compassion, a little extra #patience, and a giving heart go a long way when the holidays come around. Remain aware of what others may be experiencing, ask kind questions, and listen with an open heart and mind; that is the true holiday spirit in action. While you may be dealing with your own longings while working on #Christmas, your patients are in a similar situation with the added insult of feeling unwell and being unable to go home at the end of the shift. Although you may miss your family, you can eventually walk out the door. Cultivating #empathy for your patients’ plight is an important act of compassion.

Nursing Practice: Emotional Intelligence, Compassion Fatigue and Psychological Support!

#Nurses experience a wide range of #emotions when caring for #patients. Empathy is powerful because it builds connection. People feel heard, supported, cared for and validated when someone is taking the time to truly walk through the steps of empathy with them. Nurses are there for some of the most challenging days in an individual’s life, and have a unique ability to provide this powerful support frequently.
Patients are discharged as quickly as possible, whether from acute care or rehab, and nurses are ultimately responsible for the seamless transition of patients to home. As a specialty, #homecare has established best practices to help in this process.

The National Comprehensive #Cancer Network notes that nearly all patients experience some level of distress at diagnosis, throughout treatment and beyond into survivorship. And up to 47% of those diagnosed with a new or recurrent cancer show a significant level of distress. Unfortunately, less than half are identified and referred for #psychological support. These psychological needs must often be addressed before meeting the patient’s informational needs. #Healthcare professionals play a vital role in assessing and screening for psychological distress.

Emotional intelligence is a new concept in nursing; initial research studies indicate that #emotional #intelligence is an important part of successful nursing practice. Although research on emotional intelligence is at the developmental stage, regardless of the theoretical framework used, there is agreement that emotional intelligence includes the concepts of emotional awareness in relation to self and others, professional efficiency and emotional management.

#Compassion fatigue occurs when a caregiver experiences what experts call “secondary traumatic stress” in reaction to caring for those who are themselves suffering from traumatic events. Although caregivers don’t actually experience the event as is the case with post-traumatic stress disorder, they experience the event emotionally by caring for the patient. CF has become more widely recognized as a problem that many nurses face.
